Our health form is completed in Ultracamp (our registration portal) after registering a camper. It should be submitted at least 2 weeks prior to camp, but preferably sooner! To fill out your camper's health form, you'll need a copy of their vaccination record, Covid vaccination card, insurance card, and the camper's completed physical form. You can download the current required physical form here. Please take it with you to your camper's physical and have the camper's primary care provider complete it.
